Information to submit before your initial appointment
Please note, that when you book a consultation with me, you will be asked to complete a pre-appointment questionnaire. This must be completed and returned at least 2 working days prior to our appointment. This allows me to tailor your appointment, and resources, to make best use of your 50 minute consultation time.
Additional information, such as blood results or recent/ relevant investigation results from your GP &/or Consultant would also be useful in getting the most out of your appointment.
Payments
Payment transaction receipt and a full formal receipt, suitable for reclaiming from health insurance & also revenue.ie is provided for each appointment.
Payment is taken when you book your appointment via a secure online payment system.
A non-refundable admin charge of €7.50 is included in your appointment fee.
Cancellation Policy
A strict 48-hour cancellation period is in operation. The full fee for your appointment will be applied for late cancellation.
