Pre-appointment questionnaire.
When you book your first consultation, you’ll be asked to fill out a short pre‑appointment questionnaire. This helps me understand your goals, health background, and what you’d like support with. Having this information in advance means we can make the best use of your 50‑minute session.
It’s also helpful if you can share any recent blood tests or relevant results from your GP or Consultant. These details give a fuller picture of your health and help ensure your nutrition plan is safe, appropriate, and tailored to you.
Payments
Payment transaction receipt and a full formal receipt, suitable for reclaiming from health insurance and revenue, is provided for each appointment.
Payment is taken when you book your appointment via a secure online payment system.
A non-refundable admin charge of €7.50 is included in your appointment fee.
Cancellation Policy
A strict 48-hour cancellation period is in operation. The full fee for your appointment will be applied for late cancellation.
